Eligibility

1. Application can be made by a Fiji citizen under the following categories;

• Expired passport
• Lost passport
• Mutilated/Damaged Passport, and
• Is unable to apply for a Certificate of Identity the nearest Fiji Missions abroad.

1. All documents must be submitted and adhered to as Documentary Requirements

• The applicant is travelling to Fiji only.

Criteria

1. Application form must be filled by the Applicant ONLY;
2. In the case where the applicant is unable to complete the Passport application form, a third party may complete the form and submit a completed statutory declaration form signed by a Justice of Peace (JP) OR Commissioner of Oath confirming completion of the application form on behalf of the applicant.

• The Department reserves all rights to acquire additional documents for further verification, as and when required;

1. Where a question in a particular section does not apply to you please put NONE or N/A;
2. Providing false information in this application can lead to imprisonment or a fine or both;
3. The principal applicants spouse and dependent children to be included in the same form (applying at the same time);

• Applicant may authorize a third party to liaise with the Department on the payment of fees of FJD$133.00 in Fiji or it can be paid upon arrival in Fiji.

You must:-

1. complete prescribed passport application form (Form No.:) ) click here;
2. submit certified copies of all documents;

• keep a copy of your application and all attachments for your own record; and

1. translated certified copies of all non-English documents in English (registered translator).
