Eligibility

1. Application can be made by a Fiji citizen under the following categories;

• Elderly applicant (over the age of 60) unable to appear in person due to health conditions
• Fiji citizens residing abroad where there is no Fiji High Commission or Embassy or due to circumstances such as work, health or location.
• Applicants admitted at the hospital or with severe physical disability.

Criteria

1. Application form must be filled by the Applicant ONLY;
2. In the case where the applicant is unable to complete the Passport application form, a third party may complete the form and submit a completed statutory declaration form signed by a Justice of Peace (JP) OR Commissioner of Oath confirming completion of the application form on behalf of the applicant.

• The Department reserves all rights to acquire additional documents for further verification, as and when required;

1. Where a question in a particular section does not apply to you please put NONE or N/A;
2. Providing false information in this application can lead to imprisonment or a fine or both;
3. The principal applicants spouse and dependent children to be included in the same form (applying at the same time);

• Applicant will only be required to pay applicable passport fee once waiver approval has been issued.

You must:-

1. complete prescribed passport application form (Form No.:) ) click here;
2. submit photographs less than 6 months old, good quality colour, full-face view of your head and shoulders and taken against a plain or light coloured background;

• submit certified copies of all documents;

1. keep a copy of your application and all attachments for your own record; and
2. translated certified copies of all non-English documents in English (registered translator).
